AFFIDAVIT OF JANE SMITH
Landlord and Tenant Board — Province of Ontario
I, Jane Smith, of 123 Maple Street, Unit 4B, Toronto, Ontario, M5V 2T6, make oath and say as follows:
1. I am the tenant of the above-noted residential unit pursuant to a written lease agreement dated March 1, 2024, with the landlord, Robert Chen.
2. On June 15, 2025, the landlord retained a damage deposit of $1,850.00 without lawful justification, contrary to the Residential Tenancies Act, 2006, S.O. 2006, c. 17.
3. I vacated the rental unit on May 31, 2025, leaving it in a clean and undamaged condition, as documented in the attached photographs.
4. Despite written request dated July 2, 2025, the landlord has failed to return any portion of the deposit.
